We drove from Colorado beginning
of Oct. to the Tecote border crossing, pulling a small trailer.
Customs pulled us over and asked us to open the trailer. We were
asked to unpack the trailer. It was chock full, with stuff in every
crevice. We unpacked a 1/3 of the trailer. The customs officers
rooted around in each bag. Then a handler with a sniffer dog came
along sniffed inside the trailer, over every bag on the sidewalk,
around and under the car! Then they left. We loaded up the trailer
and were waved through.

BALL CAMP NEEDS HELP! Calling out
to all Northwest drivers. We need help getting the baseball
equipment to the Eastcape. If you are driving down and have room for
some stuff, we would greatly appreciate it. I am just talking about
bats, mitts, balls, catcher gear and batting helmets. It can be
squished into empty spot or used for padding for your stuff! We are
heading down Thursday and quickly discovered that we don't have much
room!
If you can bring things it will be greatly appreciated. We have not
ever had any issues bringing in the baseball equipment. We can
arrange to get stuff to you, or meet you ourselves after we get back
Dec 4th. Kiters - if you are driving down, we would appreciate the
help with transporting gear.
Thanks for helping! Please contact Chris at
jonandchris@juno.com
for more details and to make arrangements.

ANSWERS - DISCUSSIONS-
This category must have name and/or email address.
a. PROCEDURE: iPA is available at St. Lukes. Amerimed and the
new hospital; all in San Jose.
CONTACTS: Dr. Mercado, gerontologist, at Amerimed; Dr. Luis
Cardenas, orthopedic surgeon, at new hospital. (www.hmas.mx
)
Here is a response from one who works in this procedure:
I am an almost retired clinical pharmacist who has prepared tPA a
number of times. As you know there are a number of things that have
to be done before the med can be administered. The biggest issue is
determining the nature of the stroke: from a bleed or an ischemic
event from a clot. Obviously, if it is the former than the drug
cannot be used. Other issues are age, blood pressure (too high and
the med cannot be given), recent surgery, and history in terms of
stroke. Also the timing in relation to onset of symptoms: Up to 4.5
hrs. The medication is fabulously expensive, has to be reconstituted
and dosed according to patient weight and then given intravenously
in a series of a bolus followed by a drip. So even if a patient can
get to a hospital in the time window there is a delay in terms of
obtaining a history, vital signs and getting a weight along with
various labs.
I visited the Activase web site and learned that there has been
efforts to set up "telestroke" sites in Mexico where evaluation may
be done remotely...in fact the hospital I worked at in Medford OR
was connected to a stroke center in Portland so diagnosis and
recommendations could be accomplished accurately and timely.
I wonder if the hospital in La Paz has such a link. I think it is a
teaching hospital so I would begin an investigation there. Also the
Activase web site may be able to link you with resources in Mexico.
Finally, if I came down with symptoms and were eligible, I would opt
for the drug. However recent literature has cast some doubt on the
safety and efficacy of the drug...especially when one month
mortality after administration is considered.
It seems prudent to have extremely good medical insurance due to its
expense. One respondent said that ICU full services were $10 to
11k/day for three days in St. Lukes. Forget it at my age.
It would be well to educate your acquaintances with stroke
indications and have an emergency strategy available. Am inclined to
believe that private transport would be more expedient than the
local ambulances in this emergency.
"I know from my friend Catherine's stroke that is it helpful to have
someone call Char, she alerts the ER, whichever one, St Luke's or
Amerimed of the patient's arrival, presenting concern and condition.
The University of Chicago gives a good overview of the procedure:

Phone scam alert!
I got a call today from someone pretending to be one of our
neighbours coming down for an unscheduled visit and they were in La
Paz. A few hours later he called back and said he had hit another
guy’s car and needed money to pay him so the police wouldn’t be
called. He said that he only had travellers checks and could I pay
money into Oxxo so they could just finish with the guy and continue
on down. A few things were off and I was suspicious although this
guy knew both my cell and home numbers. I texted him and asked him
what my Husband’s name was and how many pets did I have to prove it
was really him. After that I received a couple more texts saying how
hurt he was that I didn’t believe him after we had known each other
for so long. I eventually called a friend in the States who told me
that they had spoken to our neighbour just this morning and he was
at home in the U.S. not here in Baja!
Please be wary of any calls you receive like this. The number called
from is 55 914 24289 and the account number they wanted money paid
into was BANAMEX 52041 650 0944 0391.
